Food Permits
Massachusetts Boards of Health enforce 105 CMR 590, based on the 2013 Food Code and 2015 FDA Amendments, otherwise referred to the Merged Food Code. All food service establishments should be familiar with this code and have a copy or access to the code on the premises.
Inspections
Local Boards of Health (BOH) are required to inspect food establishments, typically two times a year. This Pre-Inspection Checklist includes recommended steps that each establishment take to be ready for routine inspections.

Required Documents
- Filled out Food Permit Application
- Completed Workman's Comp Affidavit
- Water sample results that includes E-Coli and Total Coliform if water is not from a public water supply.
- Food Manager Certificate
- Allergen Awareness Certificate (if serving food for immediate consumption)
